1. Player cannot have any outstanding fees with prior team. All uniforms must be turned in to previous team before applying for new team

2. New player must turn in one passport picture (no sunglasses, babies, etc. in photo). Can not be on photo paper, CVS drugstores have passport pic facilities

3. Must fill out USClubsoccer registration form, completely, in black ink. Forms must be physically turned in w/pic, money, new team roster to Jennifer Sullivan. Forms available thru team manager or on team forms page.

4. Fees for registration are $33-. $23- USClub registration fee and $10- field fee to registrar-Jennifer no later than Sunday evening. You may also deposit complete sign-up in metal box on ALSO bin at Columbia. Include in envelope: picture, form,liability release, $$, and updated roster.

5. Team rep must turn in packet Sunday by 3pm in order for player to be eligible with CSAS card to play the following Sunday.

6. Any registration material deemed unofficial will result in player card being delayed by one week.
