USALSO WOMEN'S SOCCER LEAGUE


REGISTRATION REQUIREMENTS

1. Player cannot have any outstanding fees with prior team. All uniforms must be turned in to previous team before applying for new team

2. New player must turn in one passport picture (no sunglasses, babies, etc. in photo). Can not be on photo paper, CVS drugstores have passport pic facilities

3. Must fill out USClubsoccer registration form, completely, in black ink. Forms must be physically turned in w/pic, money, new team roster to Laura Warfield or Linda Finn Valentine.Forms available thru team manager or on forms page.

4. all players to be dropped or transferred must have player card turned in and $5- fee .

4. Fees for registration are $35- to registrar-Laura no later than Monday evening. NO registration,etc in the metal box at Columbia-all registration must be droped off and picked up. Include in envelope: picture, form,liability release, $$, and updated roster.

5. All dual card fees are $35-

6. Team rep must turn in packet to designated place by Monday evening...in order for player to be eligible with USClub Soccer- card to play the following Sunday.

7. Any registration material deemed unofficial will result in player card being delayed by one week.
